Is Sacroiliac (SI) Joint Pain Causing Your Lower Back, Buttock or Hip Pain?

H2 SUBHEADING IN THE FORM OF A QUESTION

Do you have lower back pain that sits just off to one side, near the dimple of your low back or buttock? Does it hurt to stand up from a chair, roll over in bed, walk, or climb stairs? If so, your pain may be coming from the SI joint. Luckily, there are safe and effective non-surgical treatments that can provide long term relief.

What Is the SI Joint?

The sacroiliac (SI) joints connect the sacrum (base of the spine) to the pelvic bones (ilium) on each side. These joints are small but powerful, carrying the weight of the upper body into the hips and legs. They are stabilized by strong ligaments and muscles and work together with the
spine to support every step you take.

When the SI joint becomes inflamed, irritated, or unstable, it can cause deep lower back and buttock pain that can mimic disc, muscle or sciatica pain.

Why Does the SI Joint Become Painful?

SI joint pain can develop due to:

  • Joint instability from injured, stretched or weak ligaments
  • Inflammation from overuse, arthritis, or trauma
  • Pelvic asymmetry from leg length differences or posture imbalance
  • Pregnancy or childbirth (ligaments naturally loosen)
  • Compensation from spine problems such as degenerative discs
  • Lumbar fusion surgery (extra stress shifts to the SI joints)

When the SI joint becomes irritated, surrounding muscles tighten, contributing to chronic pain and stiffness. The nearby spinal nerves can also become inflamed, causing symptoms of sciatica (leg pain and numbness).

Common Symptoms of SI Joint Pain

Patients often describe SI joint pain as:

  • Pain on one side of the lower back or buttock
  • Pain that may radiate into the hip, thigh, or groin
  • Pain worse with standing, walking, stairs, or getting out of a car
  • Sharp pain when rolling in bed or standing from sitting
  • Stiffness in the lower back or pelvis
  • Feeling like the back or pelvis is “misaligned”, unstable or weak
  • Sciatica – pain and numbness down the leg

How Is SI Joint Pain Diagnosed?

Diagnosis begins with a detailed history and physical exam. A spine and SI joint pain specialist performs specific stress tests on the SI joint (such as Gaenslen, FABER, compression, and thigh-thrust tests) to reproduce pain from the joint.

Imaging may include:

  • X-ray or MRI – to assess joint degeneration or inflammation
  • Diagnostic SI joint injection – may be used to identify if the SI joint is a pain source

Treatment Options for SI Joint Pain

We treat SI pain by restoring stability, reducing inflammation, and addressing the root cause—not just masking pain.

Non-Surgical SI Joint Treatments (Offered in Estero, SW Florida):

  • Ultrasound and X-ray Guided Injections – of biologics and other regenerative medicine solutions
  • Prolotherapy – strengthens loose SI ligaments to stabilize the SI joint and supporting structure
  • Platelet-Rich Plasma (PRP) Injections – reduces SI joint inflammation and repairs the supporting ligaments. In clinical studies, PRP has been shown to be more effective than steroids at treating SI joint pain and providing long term results. (will need the reference)
  • Bone Marrow Stem Cell Therapy – promotes deeper ligament and joint healing
  • Pelvic stabilization rehabilitation – strengthens glutes, core, and pelvic muscles for long-term results (works best when combined with regenerative injections)

Frequently Asked Questions

SI joint pain is usually felt low in the back, near the pelvis or the “dimples” at the base of the spine. It can also radiate into the buttock or thigh. Pain often worsens when standing up from a seated position or after prolonged sitting.

A detailed physical exam by a spine specialist—and sometimes diagnostic injections—can help confirm whether the SI joint is the true source of pain.

SI joint pain can be short-term if caused by a mild strain or irritation, but it may become chronic if the joint or supporting ligaments are unstable or inflamed. Without proper treatment, symptoms can last for months or even years.

Non-surgical options can be very effective for SI joint pain.

Physical therapy to strengthen and stabilize the hips, pelvis, and core is a great starting point.

Regenerative treatments such as prolotherapy or PRP (platelet-rich plasma) injections can strengthen the ligaments that support the SI joint, reduce inflammation, and provide long-term pain relief—helping many patients avoid surgery.

Yes. Prolonged standing, walking, or sitting—especially on one side—can aggravate SI joint pain. Pain often flares when transitioning from sitting to standing or after remaining in one position for too long. Taking frequent breaks to stretch or move can help reduce stress on the joint.

Sacroiliitis refers to inflammation of the sacroiliac (SI) joint. It can result from trauma, repetitive stress, or autoimmune conditions such as ankylosing spondylitis.

While all sacroiliitis causes SI joint pain, not all SI joint pain is due to sacroiliitis—some may stem from ligament laxity, instability, or degeneration.

To prevent SI joint pain from recurring, focus on strengthening the muscles around your hips, pelvis, and lower back, and maintaining flexibility in the hip joints.

Treating and strengthening the supporting ligaments through prolotherapy or PRP can restore joint stability and help prevent future flare-ups.

Yes. After a lumbar spine fusion—especially at L4-L5 or L5-S1—the SI joints often take on extra stress to compensate for lost motion in the fused segments. Over time, this can lead to SI joint degeneration, inflammation, and pain.

An MRI is not always needed to diagnose SI joint pain. Diagnosis is typically based on a detailed physical exam and specific movement tests that reproduce symptoms.

In some cases, X-rays or diagnostic SI joint injections may be used to confirm the diagnosis.

Yes. A leg length difference or hip imbalance can increase stress on one or both SI joints, leading to inflammation and degeneration.

When the hips or pelvis move unevenly, the SI joint must absorb more load, which can cause chronic pain. Correcting alignment and strengthening supporting muscles can help restore balance and reduce symptoms.

Why Choose Integrative Rehab Medicine for your SI Joint Treatment?

At Integrative Rehab Medicine in Estero, FL (serving Southwest Florida), we do more than mask pain—we fix the root cause of SI joint pain.

✅ Non-Surgical, Long-Term Relief from SI Joint Pain

Steroid injections only mask pain temporarily. Our regenerative treatments repair and stabilize the SI joint by targeting inflammation and ligament damage—without surgery.

We treat the entire SI joint complex, not just the joint space, including:

  • Joint capsule
  • Supporting ligaments
  • Pelvic stabilizers

This restores true joint stability and delivers lasting pain relief.

✅ Whole-Body Evaluation for Better Results

SI joint pain rarely exists alone. We identify why your SI joint became painful in the first place by treating related regions that overload it:

  • Hip joint (arthritis, mobility loss)
  • Lumbar spine (degenerative discs, facet pain)
  • Pelvis & core (weak stability)
  • Knee & ankle alignment (gait problems)

Correcting these imbalances prevents flare-ups and improves long-term outcomes.

✅ Precision-Guided Injections + Highest Quality Regenerative Medicine

All treatments are done with ultrasound and fluoroscopy (X-ray) guidance for precise and safe injection placement—no blind injections ever.

We also process all PRP and regenerative biologics in our on-site sterile lab, allowing us to:

  • Create high-concentration PRP
  • Customize treatments for discs, joints, and ligaments
  • Maintain strict quality and sterility
  • Deliver consistent, superior results

Better accuracy + better biologics = better outcomes.

I heard about discseel through researching my aches and pains on google, and was happy to see that a veteran friendly doctors office with a veteran representative is there to guide us through getting set up, and then getting to have a consultation as well over the phone…. I’m out of state , so it really helped in getting set up, and talk about the procedure and any concerns. The team at integrative rehab listens to your concerns, and also felt very taken care of, and on top of it they take care of their veterans which was definitely felt! If you are having back issues, and are a veteran I wouldn’t hesitate to have Dr. Sebastian and their team look things over to see if they can get you feeling good again! Kevin
After dealing with neck and back issues stemming from 21 years as a Naval Helicopter Aircrewman and trying multiple treatments and interventions, the Discseel procedure has been the most favorable treatment I've experienced. I was referred to Dr. Sebastian through the VA Community Care Program, and I am grateful that I was connected with him and his team. Dr. Sebastian, at Integrative Rehab Medicine, has been outstanding throughout the entire process. He is professional, knowledgeable, and takes the time to thoroughly explain every aspect of the procedure, recovery, and expected outcomes. He has always addressed my questions and concerns with patience and honesty, which gave me confidence in my treatment decisions. I also want to recognize Suzanne, Floyd, Michelle, and Kristine. From my first interaction with the office through follow-up care, they have been welcoming, professional, and genuinely caring. They made me feel like family rather than just another patient. Their positive attitude, responsiveness, and attention to detail made every visit a pleasant experience. The procedure has provided meaningful improvement in my symptoms and quality of life. While I still have some lingering issues that I continue to work through, I am grateful for the progress I've made and for the exceptional care I received from Dr. Sebastian and his entire team. I highly recommend Dr. Sebastian and his staff at Integrative Rehab Medicine to anyone seeking treatment options for neck or back conditions.

I had a 3 year old major herniation in my L4-L5 and medium herniation in my L5-S1 discs. I tried everything - physio, chiro, decompression, but it seemed like everyone just wanted me to keep coming back and I constantly kept getting reinjured and honestly a bit fatigued. I was finally contemplating surgery because I was not able to carry out daily life. I finally found Dr. Sebastian on youtube and reached out. He was great in the initial virtual consult. Then I flew in from Canada and went for an ozone injection in both discs. That helped. A few months later I went in for discseel with exosomes, and I feel like a new person. Each time they also included the PRP along the facet joints which I am sure will have their own benefits. I stayed on top of my rehab and workouts, especially walking everyday. I am still not back to 100% but being back to 80% is still amazing considering I was having a hard time even walking, sitting, standing, or picking up my kids. Now I am back at the gym and slowly rebuilding my pre-injury strength. There are small flare ups but they go away in a day or two, and I am learning to avoid certain workouts and movements. Dr. Sebastian and his team including Michelle, Kristine, Amanda, Floyd, and a few others I didn't get the names of, have been fantastic to work with. They answered all my questions to my satisfaction, and provided the best advice I received. I also found their team had practical knowledge around back mechanics and provided me with resources the doctors I saw before were not even aware of. This was money well spent, and I would recommend it to anyone with annular tears and herniations.

I fractured a vertebrae in my neck while in the Army back in 1990 and after working as an Ironworker by trade, my neck and back were both shot. I’ve done all the things: physical therapy, chiropractic adjustments, massage, injections and even had back surgery. After finding Dr. Klisiewicz, and having the Discseel procedure done on both my neck (4.7.26) and my back (5.13.26) I couldn’t be more pleased. The experience has been nothing short of PERFECT! Veterans have priority scheduling and they even have a veteran advocate/representative (Jason K) right there in their office to help facilitate the VA process. This made the process of getting VA approval quick and almost too easy. This procedure could be life changing for so many veterans who are suffering in pain. I encourage you to not wait and reach out today!

⭐⭐⭐⭐⭐ Outstanding Care for Veterans – Life-Changing Results As a combat-wounded Iraq War veteran who served 29 years in the United States military, I have experienced many healthcare systems throughout my career and since my retirement. Without hesitation, I can say that Integrative Rehab Medicine provides the finest VA-covered medical care I have received in the last nine years. I would like to express my deepest gratitude to Dr. Sebastian Klisiewicz, his exceptional medical team, and Jason "JK" Kinnard, Chief Veteran Officer and Veteran Care Navigator, for their unwavering dedication to veterans and their families. Because of wounds and injuries sustained during my service in Iraq, I have suffered from multiple herniated discs in my spine as well as numerous joint-related conditions. Over the years, I underwent several surgeries, yet I continued to live with chronic pain that significantly affected my quality of life, sleep, mobility, and daily activities. From my very first interaction with Integrative Rehab Medicine, I knew I was in the hands of professionals who genuinely care about veterans. Dr. Klisiewicz took the time to thoroughly evaluate my conditions, explain treatment options, and develop a comprehensive plan focused on long-term improvement rather than temporary relief. After only two procedures—and while still in the recovery process before additional recommended treatments—I can already feel a remarkable difference. My pain levels have decreased significantly, my mobility is improving, and for the first time in years I am sleeping better and experiencing real hope for continued recovery. What truly sets this organization apart is not only their medical expertise but also their commitment to guiding veterans through the VA process. Jason "JK" Kinnard has been absolutely outstanding in helping navigate the system, ensuring that veterans receive the care they deserve with professionalism, compassion, and efficiency. His dedication to serving fellow veterans is extraordinary. Dr. Sebastian Klisiewicz and his entire team exemplify excellence in patient care. They listen, they care, and they deliver results. They have helped me achieve progress that I had not experienced despite years of treatment elsewhere. Thank you, Dr. Klisiewicz, Jason "JK" Kinnard, and the entire Integrative Rehab Medicine team for your outstanding service to our veteran community. You are making a real difference in the lives of those who have sacrificed for our nation. I had a lower back procedure performed with Interactive Rehab Medicine through the VA. The procedure "Discseel was a success but the VA side trying to get approved was a bumpy road and it's not the VA per say but the wrong people are in the process for approval and that needs to be fixed. I am healing well and the Staff from Interactive Rehab Medicine were very professional and I would recommend this procedure to any veteran with lower back issues in every aspect of outpatient and quick recovery. I still have 1 more month to go to completely heal so we will see. This procedure has made a difference in my life. Thank you Jason Kinnard, Dr. Sebastian Klisiewicz and all the Interactive Rehab Staff for a great job. Keep up the good work and I will be spreading the word. Are there any T-Shirts?

I have found Dr. Klisiewicz to be a thoughtful, attentive, accurate, and highly skilled diagnostician/clinician. He performed a spinal disc procedure for me ("Discseel") which resulted in a remarkable degree of improvement in longstanding low back pain with minimal invasiveness, no complications, and a quick recovery. I completely trust his judgement, advice, and procedural capability. My experiences with he and his staff have been reliably positive. I recommend him highly and without reservation.
